Day 21: The turning point.

Days 1 to 3

The fog

Anesthesia clears slowly, swelling peaks, and most patients feel deeply fatigued. Pain is typically moderate and well-managed with prescribed medication. You will be moving only short distances — bathroom, meals, brief walks — while sleeping face-down or on your side on a dedicated BBL pillow. Drainage from liposuction sites is common and expected.

Days 4 to 7

First wave of movement

Lymphatic drainage massage begins, usually between day 3 and day 5 once your surgeon clears it. Bruising darkens before it fades. Appetite returns. You will feel tight in the flanks and lower back — that tightness is the compression, the swelling, and the lipo tracks healing together. Sitting remains off-limits except with a BBL pillow for essential tasks.

Weeks 4 to 6

Stabilization

Bruising fades, numbness in lipo areas begins to return in patches, and your energy is close to baseline. Compression transitions from stage 1 to stage 2 garments. Sitting without a BBL pillow is typically cleared between weeks 6 and 8, but only with your surgeon's approval. Fibrosis management becomes the priority of every massage session.

Months 2 to 6

The long settle

Your fat graft take is now largely determined. Residual swelling can linger for up to six months, especially in the lower abdomen and flanks. Scars soften and flatten. Patients who stay consistent with massage, hydration, protein intake, and garment compliance through this window see the cleanest, most lasting results.
