Days 1 to 3

The fog

Anesthesia clears slowly, swelling peaks, and most patients feel deeply fatigued. Pain is typically moderate and well-managed with prescribed medication. You will be moving only short distances — bathroom, meals, brief walks — while sleeping face-down or on your side on a dedicated BBL pillow. Drainage from liposuction sites is common and expected.

We speak directly with your surgical coordinator before you land. Discharge instructions, medication list, compression schedule, and follow-up dates are integrated into your care plan before you check in.

No-sitting support

BBL pillows are staged at every location you will use — bed, dining, transport vehicle. Staff are trained to prompt repositioning every two hours during waking hours to protect the graft and prevent pressure points.

Lymphatic drainage cadence

Sessions begin once your surgeon clears manual massage, typically day 3 to 5. We run a tighter cadence in the first 10 days — often daily or every other day — then taper to two or three sessions per week as swelling subsides.

Compression and garment management

We adjust your faja and lipo foams daily, watch for skin compromise under seams, and coordinate the stage 1 to stage 2 transition with your surgeon. A poorly fitted garment is one of the most common — and preventable — causes of asymmetry.

Fibrosis prevention

Targeted manual work on the upper abdomen, flanks, and lower back starts early to break up the firm, ropey tissue that follows liposuction. Ultrasound and radiofrequency modalities can be added as indicated.

Discharge and travel readiness

Before you fly home, we confirm you have a BBL pillow for the plane, compression for the flight, a written massage plan to continue locally, and a flagged list of anything your home provider should watch for.

We want you informed, not alarmed. If any of the following happen during your stay, tell a staff member immediately — we will loop in your surgeon. If you are already home and see any of these, call your surgeon the same day or, where noted, call 911 first.

  • Shortness of breath or sudden chest pain

    Can indicate a pulmonary embolism, which is the most serious known risk of BBL surgery. Treat this as an emergency — call 911 before calling anyone else.

  • Fever above 101 F (38.3 C)

    A low-grade fever in the first 24 to 48 hours can be normal. Sustained or rising fever is not. Contact your surgeon the same day.

  • Calf pain, redness, or swelling in one leg

    Possible deep vein thrombosis. Do not massage the area. Call your surgeon and seek urgent evaluation.

  • Asymmetric, rapidly growing swelling in one buttock

    May indicate a hematoma or seroma requiring drainage. Contact your surgeon within hours, not days.

  • Foul-smelling drainage or darkening skin over a lipo site

    Signs of possible infection or skin compromise. Do not wait to see if it improves overnight.

  • Persistent vomiting or inability to keep fluids down past day 2

    Puts you at risk of dehydration and interferes with healing. Let your team know so we can coordinate with your surgeon.

  • Numbness that spreads instead of fading

    Some numbness is expected and resolves over months. Numbness that increases in area, or is paired with weakness, warrants a same-day surgeon call.
